Initial Consultation

After contacting our office for an initial consultation you will receive a welcome packet that includes a thorough "new patient" form. This form may be longer than you have had elsewhere.  It is very valuable in your initial consultation.  It is designed to maximize information we need for a proper diagnosis and eliminate any delays in your diagnosis and treatment plan. Please download or printout this form and take your time in filling it out before your appointment.  Your initial appointment is reserved exclusively for you; we do not double book patients. Patients rarely wait more than five minutes to be seen. We respect your time.

At your initial consultation we will review your medical and dental history. Any anxiety or concerns you have will also be explored. An intraoral cancer screening will be done along with a periodontal and complete tooth-by-tooth restorative exam.  Necessary radiographs will be taken and interpreted by the doctor. Teeth will also be evaluated with Diagnodent, Electronic Delay Detector, and shown to you on an intraoral camera. All necessary diagnostic equipment is located at our facility. Cosmetic imaging is also done if needed to allow you to see what your smile can look like before any work is done. Study models are also made if needed.  Once the doctor has gathered all necessary information, it is discussed with the patient and a treatment plan is designed specifically for you.

Please assist us by bringing the following information to your consultation appointment

  • Your new patient form and copies of any recent X-rays, if applicable
  • Referral slip from another doctor, if you have one.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary filled out forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under 18 during the consultation visit.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves or joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) If you are required to premedicate before a visit to the dentist, please inform us.
